Member of the hospice staff who determines appropriateness of referrals, coordinates and conducts preadmission activities, monitors inpatient care at contracted hospitals, performs in-service training and community education activities.
*Increases accessibility to hospice care, to manage the hospice admission process via effective communication.
*Describes and explains hospice services to a potential patient/family and elicits responses to questions regarding patient/family attitude toward hospice care, physical and psycho/social/emotional status, in order to put the patient and family at ease about choosing hospice and fully inform them about the program.
*Gathers pertiinent information necessary for admission.
*Speaks to patient/family about insurance coverage and financial obligations, using knowledge of hospice fees, cost and reimbursement sources.
*Promote hospice services, to ensure appropriate referrals to hospice programs, costs and reimbursement sources.
*Promote hospice services, to ensure appropriate referrals to hospice program to discharge planners, physicians, other health care providers and referral sources.
*Performs liaison activities during the hospitalization process.
*Promotes knowledge and awareness of the hospice program and its services to the professional community.
Registered nurse with current license in Massachusetts and New Hampshire. Demonstrates knowledge and experience in home health care and in hospice care. Excellent interpersonal skills.
