    The Hospice Social Worker provides direct service to patients and families. Participates in a multi-discipline case conferences and consults to agency care providers regarding social service needs of clients and families in Dover, Sommersworth, Rochester and Rollinsford NH.

    Responsibilities

    • Provide psychosocial and financial-environmental assessments related to patient's illness, needs, response to treatment, the patient's financial resources, and availability of community resources.
    • Develop a care plan and coordinate this plan in collaboration with the physician and nurse or therapist.
    • Evaluate the impact of the care provided
    • Provide counseling services to patients to assist the patient to cope with his/her medical condition and to respond to treatment
    • Provide short-term therapy with goal-oriented interventions directed toward management of terminal illness, reaction and adjustments to an illness, and resolving conflicts related to the chronicity of an illness
    • Provide Community Resource Planning by establishing the need for and making referrals to community resources such as supplemental health care, meals on wheels, food stamps, advocacy and linkages
    • Provide long-range planning by assessing the patient's needs for long-term care, including evaluating the home and family situation, helping the patient and family to develop an in-home care plan, exploring alternatives to in-home care and arranging for placement.
    • Consult with other team providers to assist them in understanding significant social and emotional factors related to the patient's health problems.

    Qualifications

    • Master’s degree in social work from a school accredited or approved by the Council on Social Work Education.
    • At least one year of social work experience in a health setting.
    • LICSW current licensure NH or willing to get NH license.
    • Hospice experience perferred.
