Are you looking for an interesting and dynamic position in the growing field of home health care? 


As a Behavioral Health RN/Registered Nurse candidate you will perform comprehensive patient/family assessments of mental status, respond to treatment, and establish an effective plan of care based on this assessment. This position provides nursing services to Psychiatric/Behavioral Health clients and families in the community via a primary care nurse model. 



**Offering $2,500 Sign On Bonus**


  • Performs on-going comprehensive patient/family assessments which includes patient’s mental status, response to treatment, patient’s ability to care for self (and others), patient’s and family’s coping abilities.
  • Demonstrates appropriateness in involving the patient and the patient’s family in the assessment.
  • Provides supportive/validative psychotherapy crisis intervention.
  • Educates patient and family regarding early signs of relapse.
  • Documents medication education including recommendations, assessments of efficacy and monitoring for side effects.
  • Provides linkages between institutions and community based care.
  • Establishes behavior plans.
  • Promotes therapeutic strategies.
  • Assesses spiritual issues providing community referrals when appropriate.
  • Assesses safety (environmental, physical and mental) and provide appropriate recommendations when deficiencies are identified.
  • Incorporates standardized measures and best practice interventions.
  • Recognizes professional limitations and seeks consultation appropriately.
  • Works cooperatively with Psycho Social Program Manager to implement community Psychiatric/Mental Health program and to provide patient/family services.
  • Keeps current with Psychiatric Nursing Practices.


  • Active Registered Nurse/RN in MA and NH (or willing to obtain NH license).
  • Recent acute care psychiatric nursing experience desired.
  • BSN preferred.
