The Medical Records Scanning Clerk is responsible for the confidentiality, maintenance, and tracking of all medical records and related

documents for all companies of the Home Health Foundation.

Responsibilities

  • Accurately prepares patient records for scanning into the optical disk system, ensuring that all staples and paperclips are removed and that each document is properly labeled. 
  • Accurately scans and identifies documents, ensuring that the correct data is attached to each image. Performs QC on documents scanned.
  • Accurately assembles admission paperwork (admit packs) by discipline and clinical specialty according to contents of admit folder lists. 
  • Assists staff with retrieving scanned documents. 
  • Accurately processes “Request for Service” from Intake Department in a timely manner. 
  • Assists clinicians with HealthWyse devices, as needed. 
  • Manages and tracks specific equipment returns. Alerts manager if equipment is damaged. 
  • Accurately differentiates between hospice and VNA referrals and scans into correct email. 
  • Demonstrates a knowledge and understanding of what to report to the supervisor or Director of Quality Improvement when concerns of corporate compliance arise. 
  • Ensures compliance within guidelines set forth by regulatory agencies (JCAHO, DPH, ERISA etc.) and demonstrates compliance with Home Health Foundation policies and procedures. 
  • Practices confidentiality principles set by the agency and federal HIPAA guidelines.
  • Participates in all mandatory inservices.

Qualifications

  • High School Diploma or equivalent.
  • One year office experience (medical setting preferred).
  • Ability to adhere to deadlines.
  • Works efficiently and accurately and is detail oriented.
  • Customer satisfaction oriented.
  • Ability to lift 25 pounds safely.
  • Good communication skills.
