Overview

Are you looking for an interesting and dynamic position in the growing field of home health care?  

 

As a Clinical Liason you will assure continuity of care from hospital to community and community to hospital. Channel verbal and written information to and from appropriate hospital personnel so that patients and families are assured a comprehensive assessment, plan and implementation of hospital and post-hospital needs.

Responsibilities

  • Work cooperatively with Hospital Discharge Planners, Nurses and Social Workers, as well as client physicians to assure patient needs are met along the continuum of care.
  • Coordinate and organize a multi-disciplinary approach to patient care planning.
  • Follow through with HomeCare Coordination for new patients assigned by Discharge Planning Personnel.
  • Demonstrate proficiency in utilizing assessment skills.
  • Assess the continuing care needs of patients.
  • Utilize the individualized systematic approach which includes the client, family, professional staff and other support services to assure that all relevant aspects of care needs are understood.
  • Establish a plan of care for providing continuing care patient needs.
  • Serve as a consultant to other care providers to assure a comprehensive approach to planning patient care.
  • Invite the participation of family and significant others in the care plan.
  • Consult, counsel and educate the patient and family on available resources needed to provide or support the care needed.
  • Participate in inservices and education on home care services within the agency and within the Hospital.
  • Participation in the development of liaison marketing strategy with both medical and Hospital referral sources.
  • Promote hospital staff participation in public activities to convey agency mission and generate visibility.
  • Document care provided and plans for continuing care.

 

Qualifications

  • Active Registered Nurse/RN or LPN license in Massachusetts.
  • Community Health experience.
  • Prefer hospital experience or experience in a health care setting.
