Overview

Are you looking for an interesting and dynamic position in the growing field of home health care?

 

Per Visit nurses wanted to provide comprehensive clinical care to patients in their homes. Evaluate patient needs at every visit and coordinate care with other team members to provide the most appropriate care for our patients. Assess and report significant clinical findings to the physician, to the primary nurse and assess the patient for ongoing clinical needs.

 

Become a part of the team by committing to just 12 visits a month with a minimal weekend commitment and no holidays! If you have at least two years of current home care experience and are looking to work in the Greater Lawrence and Salem, NH area, we’d love to talk to you more.  We provide  a full orientation program to all new employees.

Responsibilities

  • RN utilizes the nursing process in planning, providing and evaluating skilled nursing care to individuals and families. 
  • RN performs a comprehensive nursing assessment including physiological, psychological and environmental factors.
  • RN develops, evaluates, and revises patient care goals in conjunction with the patient and family.
  • RN coordinates goal directed, cost effective care for primary patients and demonstrates appropriate use of community resources  to deliver comprehensive individualized care.
  • RN communicates with MD, family and interdisciplinary  care providers to clarify care and increase awareness of significant changes in condition in a timely manner.
  • RN maintains effective working relationships with patients/families, referral sources, physicians, community resources and insurance representatives.
  • RN provides adequate education and training of patients and families. 
  • Assists with training of new nursing staff. 
  • Takes initiative in self-growth and professional development.
  • RN discharges patients when goals are met.

Qualifications

  • Must be licensed in Mass. and N.H. and have a valid driver’s license and current CPR certification.
  • Must be able to attend our orientation program.
  • Two years of community home care experience required.
