Hospice care is provided by a multi-disciplinary team that provide the highest quality care with compassion and commitment to patients and families. As a Clinical Manager, you will provide supervision and guidance to the Admission Team. Maintain and promote optimal relationships with referral sources and ensure that all referrals are handled efficiently and effectively.


  • Ensure all referrals are handled efficiently and effectively 24 hours a day, 7 days a week.
  • Monitor and measures the performance of the Admissions Team.
  • Hire staff, coach and develop admissions team, monitor staff performance for accountability.
  • Address performance issues in a timely manner, maintain documentation on employee performance and prepare/deliver balanced performance evaluations.
  • Participate as a member of the MVH, act as a team player and maintain a positive and supportive attitude.
  • Keep Director/V.P. informed to achieve goals, support grants and fundraising activities.
  • Ensures compliance with hospice regulations, attend appropriate conferences, abd CEU programs and seminars.




  • Active Registered Nurse/RN license in MA and NH (or willingness to obtain within 60 days).
  • Minimum of 2 years of relevant clinical experience, homecare and hospice case management experience desired.
  • Knowledge of current nursing standards of practice, home care, palliative and hospice regulations.
  • Advanced planning, organizational, supervisor, coordinating and marketing skills.
  • Strong interpersonal and customer relations skills.
  • Ability and willingness to self-motivate, prioritize, and be willing to change processes to improve effectiveness and efficiently.
  • Adapts to changing patient and organizational priorities.
  • Effective oral and written skills.
  • Computer literacy skills including but not limited to data entry, retrieval, and report generation.
  • Ability to work independently, while collaborating with other team members.
  • Able to provide own transportation to patients homes or current location.
