Overview

Are you looking for an interesting and dynamic position in the growing field of home health care?

 

In this position you will be responsible for supporting quality assurance audits and monitoring activities for the VNA and Hospice. Working on 485/POT/Orders and other lists at the direction of the Quality Manager. Assisting with pre/post admission reviews, supporting the function of the quality, compliance and risk departments.

Responsibilities

  • Effectively use and manage assigned health information management software tools including e-faxing, document organizers, form “scrubbing” and process/outcome data reporting.
  • Ensure correct key attributes (i.e. start of care dates, certification dates, recertification date spans, face to face encounter dates) and Healthwyse to do lists are managed and addressed.
  • Ensure electronic documents are correctly filed in electronic storage systems.
  • Scan medical record documents in the archive record system as needed.
  • Effectively use Forcura and tools to assess documentation in scanned docs for necessary clinical information to assist with correct coding. 
  • Eventually will pre-code diagnoses in EMR and referral software per department guidelines using to processes/tools developed by Quality Manager.
  • Meet/exceed expectations of efficiency, effectiveness and department cultural goals as set by V.P of Quality, Compliance and Risk.
  • Work with clinical team to problem solve issues with physician identification.
  • Actively participate and accurately complete pre-admit checklist activities (verification of eligibility, documentation of results, communication with referral sources/clinical staff and creation of supporting documents). 
  • Demonstrate ability to access and utilize Home Health Gold/SHP/Forcura/Healthwyse to assist in audits/scrubs as determined by Quality Manager.
  • Audit assigned electronic documents and forms for completeness as assigned by quality assurance nurse.
  • Demonstrate good judgment in document handling and independent problem solving relative to issue.
  • Bring appropriate issues to the attention of supervisor. Manage the VNA Hold List for timely closing of records. 
  • Assist with follow up as directed by the Quality manager for billing issues. 
  • Demonstrate ability to work within the areas of the 485 and plan of care in Healthwyse.
  • Demonstrate ability to access and utilize Home Health Gold/SHP to assist in audits/scrubs as determined by Quality Manager.

Qualifications

  • High School or greater education.
  • Minimum of two years of office related experienced.
  • Diagnosis coding experience a plus.
  • Prior experience in a medical office preferred.
  • Proficient in Microsoft Word and Excel.
  • Able to accurately data enter into computer software systems.
  • Able to learn and use Agency EMR and e-discharge software.
  • Highly organized, flexible and dependable.
  • Pleasant phone manner.
  • Congenial, positive attitude and cooperative.
  • Demonstrated ability to problem solve and meet deadlines.
