Are you looking for an interesting and dynamic position in the growing field of home health care?

 

As an Intake Data Coordinator you will manage admission and referral information, data processing and communication between and among referral sources, intake staff, clinical staff and staff of other relevant departments such as MIS, Fiscal, Quality, Utilization Review, Hospice and Homecare.

Responsibilities

  • Enter all demographics, referral source, and insurance information for referred patients.
  • Print faxes throughout the day.
  • Reconcile electronic tracking, white board and referral worksheets.
  • Distribute referrals to Intake nurses to be completed in Healthwyse.
  • Verify facility discharges daily and maintain patient log with calls made and actions taken when following up on a referral.

Qualifications

  • One to two years experience in health care setting preferred.  Experience in home health industry preferred by not required.
  • Ability to work 3 of the six agency-designated holidays.  Manager will assign coverage at the beginning of each year.
  • High school or GED required; A.S.. preferrred.
  • Certificate training or equivalent experience working with medical terminology required.
  • Demonstrated computer proficiency.
  • Demonstrates initiative, positive attitude.
  • Excellent communication skills; adaptive to change.
  • Flexibility in scheduling.
